1. Identify at least one instance in this story where you believe an error in patient safety occurred. After reading this very sad story I believe there were many errors that occurred to cause the death of that poor child. I strongly believe that as a mother that we know our children best. We may not know the how to treat serious medical problems. But, we know how our children act when they are sick, happy and so on. The first mistake made was when Josie’s mother notices she’s very thirsty and she mentions this to the nurse. In which the nurse just responded with it being a common issue in the patients she deals with. Also when her mother had noticed Josie being extremely thirsty the nurse should have checked the child instead of just considering her a common case because as we all know people are different and their bodies react different than others. As mentioned in the story of Josie her mother mentioned that every time her child would cry the nurses would just hit her pain button. I strongly believe that is wrong just filling a kid up with drugs because they didn’t want to take the time to check on her. Finally the other issue of the doctor ordering to get more drugs to the child and sending an unfamiliar nurse in to do it is wrong!!!!! The doctor should of came in and explained the order and why he decided to do what they did and me being a mother of two daughters ages 8 and 13 I would want to know everything they are doing to my children regardless of how big or small the order is. So when that unfamiliar nurse came in to give that child drugs after her mother understood that no more should have been given. The nurse should have stopped and got the nurse instead of ignoring her
