Kaiser Permanente (KP) was founded in 1945 at the height of the Great Depression (Kaiser Permanente, 2017a). Since their humble beginnings KP has strived to provide the best care possible for their patients and improve on health for their members. Kaiser has grown from a small hospital, to a large foundation by making appropriate changes and using strategic planning to ensure members receive the health care they need in terms of quality and safety. One method of strategic planning KP uses is appreciative inquiry, a method and philosophy that identifies when the facility functions at its best and builds from those situations to create a vision of the future and work towards that vision (McNamara, n.d).
Collaboration among health care professionals is defined as assuming complementary roles and cooperatively working together, sharing responsibility for problem solving and making decisions to formulate and carry out plans for patient care. (Fagin, 2008). In any field of health care where physicians and nurses interact with one another for the purpose of quality patient care, it is vital for them to work together as one and understand the needs of their patients as well as each other’s roles and responsibilities. In short, teamwork should prevail for excellent patient service. Therefore, being in full partnership as a nurse with the physician and other healthcare professional is another recommendation that is applied by the RWJ-IOM report.
An article published in October of 2015 recognized the need for a standardized handoff template such as SBAR to assist in the patient information exchange. It was noted in multiple literature reviews conducted that barriers to effective handoff communication included too little information and the inconsistent quality of information (Clochesy, M, Hickman Jr, & Gittner, 2015; Beaumont & Russell, 2012)This lack of information can have a poor impact on patient outcomes, and inversely effective communication in the entire care team improves knowledge, patient goals, and the patient plan of care (Johnson, Carta, & Thondson, 2015). Communication in nursing is known for its life saving success as well as its greatest flaw in poor patient outcomes. There is always room for improvement and when communication is carried out efficiently, healthcare professionals have reaped the benefits. However, there have been many instances in which nurses have had to learn the hard way of how detrimental communication can be to patient safety. Through research and reviews of literature, the topic of patient safety related to handoff communication among units is analyzed.
Communication is an essential piece of caring for patients. Multiple team members will collaborate when providing patient care. It is crucial that critical information is included in the numerous hand-offs that will occur. A patient and nurse interact and communicate and form an interpersonal system that is affected by situational factors in the environment (Alligood, 2013). Alligood (2013) also identifies the fundamental concepts of this theory as: perception, communication, interaction, transaction, self, role, growth and development, stress/stressors, coping, time, and personal space.
