On 1/20/2015 SO EMT Perez was dispatched to KC-304 regarding skin pain. SO EMT Perez knocked, announced his presence and was verbally invited in by the resident. The resident, a Mrs. Marian Fox stated that she has been having some severe skin irritation the last couple of hours and she wanted a second opinion on what she should do. SO EMT Perez performed an assesment which revealed the following; Blood Pressure 120/80, Pulse 64, no visible lesions or abrasions of the area where Mrs. Fox was complaining of pain and no other pain outside the ordinary. Mrs. Fox than decided to lay back in her bed.
CCIB received an Incident Report from the LTCO Maged Mansour (213) 351-6433 regarding resident Maria Alcantara. The SOC 341 states that Maria disclosed to facility staff and her Day Program staff that residential facility staff member Gloria G hit her in her back. Once notified by of incident the facility staff notified the administrator Kendal Woods (323) 208-0133. According to the RP the incident is under investigation. The suspected abuser would be suspended pending

The reporting party (RP) reported being contacted by school office of Williams Elementary 7530 Arnett St., Downey, CA 90241 (562) 904-3564. The school office reported observed what appeared to be a burn to foster child Amber Gonzales ' DOB; 11/14/08 right hand. DCF Arlene Barragan investigated. On 3/17/17 foster mother Alma Yepes took the foster child to the hospital located at 11500 Brookshire Ave., Downey, CA 90241 (562) 904-5000 for an examination, the wound was cleaned and wrapped.

My ACL Tear Journey At The Hospital A quick turn on a soccer field led me to the worst experience in my life. A while back in my sophomore year I tore my ACL while I was practicing for my first soccer game of the school year. I made a quick turn without positioning my feet correctly on the ground. I thought that I broke my knee, but I never knew that after that day I would have experienced the worst day a month after on February 15th.

Samantha Davenport would walk into class with an attitude some days. Some days she was a sarcastic little bundle. She was never just sitting down. She would roam the room, interacting with each and every student. Whether that interaction was getting in their face or tapping their shoulder to make sure they were paying attention, she was always there.
Over the past four years I have seen more doctors than an average person will see in a lifetime. From endocrinologists to neurosurgeons, I have tirelessly sought medical treatment for multiple traumatic brain injuries I incurred at a young age. Through my personal struggle I learned that there is no adequate treatments for concussions and traumatic brain injuries in the State of New Mexico. Although the lack of these services forced me to seek treatment outside my community, it has become my main motivation to return to my community and share the knowledge I have been fortunate to receive through these many experiences. At the age of fourteen, I was the unfortunate recipient of four traumatic brain injuries, within a six-month time frame, that would result in a diagnosis of post-concussive syndrome.
Surrounded by sterile white walls in a plain paper gown, I quickly glanced at the worried look on my mother’s face from my periphery. I knew for sure this was not going to end well for me. As the doctors examined my skin, all my ten-year-old mind could think of was how I had gotten out of school for the day. I was clueless as to the long-term effects this doctor visit would have on me.

Beginning at the age of nine I had issues with my knee, issues entails whenever I extended my leg or ran my knee cap would slide out of place. When I first noticed it my parents and I set up an appointment with an orthopedic doctor to look further into the problem. After a few other appointments with multiple doctors, we decided to try physical therapy and a knee brace. Various doctors wanted to do surgery but considering I was only nine, surgery was not our choice. After a while we decided to leave the problem alone and see what happened as I grew.
Injured on bulls Some people say bull riding is a fun sport but there is some consequences in the sport by you will get injured like a broken leg, arm, wrist, ribs, knee, etc… For an example i was 14 i went to Oahu for a bull riding event since i was there i sprained my knee and went to queens for some x-rays and this really impacted my learning for school and school was the next day so i stay home for 2 weeks so is was courageous that my knee will heal In conclusion i wish that i didn’t sprained my knee-cap and wish to come school to see friends and get and education
