INVESTIGATION: On 06-30-2016, I was notified at approximately 0021 hours, of a traffic crash fatality on Bearss Avenue east of Livingston Avenue. I responded to the crash scene and arrived at approximately 00103 hours. Upon my arrival at the scene, I met with, Corporal Morrill, Detective Parsons, who forensically mapped the scene and took digital photographs of the crash scene and Deputy Moraniec. Deputy Moraniec completed the Department of Highway Safety and Motor Vehicle Traffic Crash Report (6560901). I conducted an interview of Anthony Walter Chavez, the driver of the Lincoln Navigator. During the interview, Chavez refused to submit to a blood draw. During the time we spoke, I noticed no indication of impairment with Chavez. I contacted the Hillsborough County Sheriff's Office Communications Section and requested a copy of any 911 calls received in reference to this crash. On 08-01-2016, I contacted the Hillsborough County Medical Examiner Department and requested and received the initial case summary for Humphrey Naciso Felipa. The summary showed that Humphrey Naciso Felipa’s death was due to C2-C3 cervical vertabral fractures with laceration of the brainstem due to blunt impact to the head and neck. On 08-22-15, I received a copy of the 911 calls made in reference to this crash. The calls revealed two additional witnesses, Amelia Pringle and Tamoris Heard. (See interviews) On 08-31-2016, I received the toxicology report from the Hillsborough County Medical Examiner’s Office. The report showed Felipa to have a blood alcohol level of .21 grams/deciliter, (Ocular fluid) and .27 milligrams/liter of cocaine (peripheral blood), which indicates recent cocaine use. On 12-20-2016, I updated the Driver And Vehicle Information Database (D.A.V.I.D.) with the crash and decedent information.
