On 3-19-16, Highway Patrolman Jeremiah Byrd had a traffic stop. The vehicle was occupied by the alleged perpetrators and the alleged victims. The children were in restraint in the maroon Chevrolet Suburban 1500, traveling East on I10, near the mile marker 61. Judith gave the officer a Texas ID card, and it was suspended. Ramiro also have a driver license to the officer.

On 09/18/2016, at approximately 2148 hours, at the Durango Jail located at3225 W Gibson Ln Phoenix, AZ 85009 an Inmate/Inmate fight had occurred in Durango 3 B Pod between Inmate Moothery, Naz T303229 and Inmate Sams, Worine T284566. Upon entering Durango 3 B Pod, Inmate Sams approached Inmate Moothery and begun fighting by Durango 3 B Pod door. Multiple strikes were thrown by both inmates until Inmate Moothery had fallen to the Pod floor. At that time Officer Serrano approached the pod door and gave both inmates commands to stop fighting. No radio call was made due to both inmates complying with the orders given.
On 11/07/2017, the agent received a Law Enforcement Contact form indicating offender Joe A. Slugger was at the K&D Tap on 11/06/2017 having unauthorized contact with Sandra Williams. It was stated in the LEC form that Joe Slugger was drinking alcohol and will be referred for the charge of felony Substantial Battery by Badger Police Department after he was witnessed to hit Sandra Williams with his open right hand to Sandra William’s left side of her face and then grab Sandra Williams by the back of her head and slamming her head onto the bar counter where she was seated. Sandra Williams was observed having a visible red mark on her face and a laceration on her forehead that later required eight stitches. On 11/08/2017, the agent received a police report from Badger Police Department Officer Konkol (Case number 1-98-00456723) stating Officer Konkol and Officer Sandner were dispatched to the K&D Tap (223 W. main Street, Badger WI) where they made contact with Joe Slugger.
On June 18th 2016 Deputy Akins was requested by dispatch to make phone contact with Antonio Johnson regarding Nia Sockwell who had fallen on Tuesday and hit her head on a towel rack. Upon making phone contact with Antonio Johnson, Mr. Johnson explained that his cousin Nia Sockwell, had fallen and hit her head on the towel rack while at her boyfriend’s home Tuesday, June 14th, 2016. Mr. Johnson said the cousin’s boyfriend is “Al” who lives at 1304 Valentine Rd Jacksonville Arkansas. Mr. Johnson could not provide any other information on “Al”.
On the above date and time I was conducting an area check of the the Ridge Road Center plaza. While behind the plaza, I observed a white, 4 door Mercury occupied by three white males. The vehicle was located in the back of the plaza next to a closed store. As I passed the vehicle all four males continued to stare at me. Concerned that the driver, or passengers may have a medical emergency, or a disabled vehicle, I proceeded to pull behind the vehicle without my lights or sirens activated to inquire.
On 04/23/16 I contacted Andrew Jepko at Pinching Pennies in reference to a theft of property call. Mr. Jepko said on this date at around 1230 hours a white male, white female and mixed juvenile male entered the store and browsed around for a while. Mr. Jepko said the party exited the store and the group stood outside the business for a short period and then the female reentered, expressing interest in a set of tires he was selling; which were stacked in front of the business. Mr. Jepko said the female exited the business again and when she did her male companion put two of the tires in the back seat of their vehicle with the juvenile male and then the three drove away. The purpose of this report is to document the arrest of Rigoberto Garcia-Martinez (M/H DOB 08/03/85) for, driving under the influence, driving with a BAC of .08 or more, driving a vehicle with unsafe equipment and operating an uninsured vehicle. I (Trooper Castillo #6570) was on patrol at 06:30 a.m. on 05/08/2016 when I observed a Blue Chevy Cobalt with front end damage a leaking fluids traveling southbound on I55 just before the Pulaski exit. I initiated an investigative traffic stop on the blue Chevy Cobalt.

Carlus Ingram and P.P.O Harris responded to Interstate 70 Westbound exit ramp at Salisbury Street for a call for a “Department Accident.” After responding they were advised by Driver 2 that he was stationary on Interstate 70 westbound at Walnut Street relative to a call for construction debris on the highway. Driver 1 was traveling westbound on Interstate 70 approaching the rear of Driver 2 patrol vehicle. While Driver 1 was attempting to merge into another lane he side swiped the diver’s mirror of Driver 2 vehicle. All parties stated they were not
Upon my arrival to the scene at approximately 1645 hours, I observed the crash scene to be just east of Providence Road along the south side of Lumsden Road and at the east driveway entrance to the Brandon Centre South shopping plaza at 1945 W. Lumsden Rd, Brandon, Florida. Both vehicles came to rest off the roadway only the outside eastbound traffic lane was closed to traffic by Hillsborough County Sheriff’s Office patrol units. The driveway was closed to all pedestrian and vehicular traffic. Master Deputy Millovitch # 7044 had also responded to the scene and was originating the DHSMV Florida Crash Report (86200813).
On 5/25/16 at 9:40 PM, Integrity Associate (I/A) Nathan Govan (natgovan) notified Shift Supervisor Enmanuel Cabrera that he believe someone may have stolen his lunch. S/S Cabrera quickly gathered the information needed and walked over to the refrigerator at question to get the exact location where the he left his lunch. On 12/20/2015 at 00:41 hours, while traveling southbound on Hurffville Road, I observed a Black Ford bearing NJ Z47CSR failing to maintain lane. The vehicle was swerving to the left over the center lane. I activated my overhead lights and sirens and conducted the motor vehicle stop on Rte. 42, mile marker 11.7. 2. On 12/6/16 at 1700 hours, I received a call for service at 3811 E Main St, in reference to a battery that occurred. The victim, Bill Johnson, and Robert Clarke arrived at the City Theatre at 1400 hours, and went to the rear of the line for a movie. Johnson was standing in front of Clarke when an unknown subject arrived east of the property and stands in front of Johnson. Johnson tells the suspect that the line starts at the rear of the line.
