There are seven key concepts that are required in order to have effective communication. They are mutual respect, harmonized goals, supportive environment, appropriate decision partners, the right information, transparency and full disclosure and continuous learning.
Mutual respect occurs when the patient and care giver are partners during the decision making process of the patient. I see this being done during physician rounding and visits. The patient will ask about their care and the physician discusses the options and possible outcomes. The physician and the patient discuss what they feel is the best option for the patient.
Harmonized goals occurs when the patient and caregiver agree of the patient goal. The two have discussed the risks, benefits, costs and the patient goal before implementing patient care. This is done throughout the care of the patient. The patient will continue to have visits with the care giver. The physician will continue to communicate and assess the patients care and overall well being.

The three methods of communication are situational briefing guide, team headlines and multidisciplinary rounds using daily goal sheet.
The situational briefing ground, SBAR, indicates the patient situation, background of the patient, our assessment findings/what the problem is and our recommendation to the physician. This is what I was taught to use during nursing school. This tool seems to be the method of communication for all nurses that I have worked with.
Team headlines are meetings that are held at the beginning of the shift with the care teams so that everyone is on the same page.
Multidisciplinary rounds are meetings or rounds done with the entire patient care team to collaborate about patient care and
