This directness according to the scenario is a key feature of all parties to meeting the needs of Susan must be at the heart of all professionals involved in Susan's care despite the family problems. By taking the help available, Vicky has also empowered the professionals to empower her. Theories of collaborative working Effective team working requires excellent communication and interpersonal skills between professionals whether they are working directly with this family or whether they are helping to assist the professionals involved. Informed decision making Informed decision making can only occur when there has been full disclosure of all relevant facts. This will need to be from the professionals to the family and from the family to the professionals. If there is communication breakdown at any point of this process, it could result in other serious consequences for each individual involved depending on what and if this situation occurs. Information sharing It is imperative to ensure that all the involved professionals and agencies are made fully aware of all relevant information about Susan, Vicky and the rest of the family who are affected situation to provide joint care.
