Circumstance: Ms. Smalls (MHP), Mrs. Gailliard (MHS), Clarence and Ms. Elizabeth Strong (DSS Worker) schedule medical appointment with the MUSC Foster Care Clinic. Action: MHP called Tara Peevy, RN at the MUSC Foster Care Clinic after MHS explained leaving several messages. Ms. Strong explain emailing the referral form to the clinic. Machelle Green explain receieing the referral form, however unable to reach the DSS worker for additional infromaiton.

On 09/29/2015 at 3:18 PM SC received call from Marguerite Pa’s niece who reported that Pa has identified two girls Gabby and Yajaira he wants to be his aide but the agency told them his service were terminated. SC attempted to explain why the service was interrupted and next step; SC end call with Marguerite. SC then called Vital Support and briefly explained to receptionist and requested to speak with a manager. SC was transferred to Vitaliya at Vital Support Nurse manger. Vitaliya stated that the termination date is incorrect and service was provided to Pa beyond termination date and that SC need to change the dates so that they can receive payment.

The referrals were not received or processed, however you completed a Case Management Visit on 4/1/16 regarding information contained on the referrals. How would you like me to handle the referrals. Medicare originally denied the (expensive) prescriptions for the suffering patient. The pharmacist then called the insurance to get authorization and was told to wait for a call back. Days later, an adjuster from Medicare called the pharmacy and told them to rerun the prescription. The adjustor also said that the nebulizer the pharmacy had in stock would not be covered and that they had to order a different one for the following day(Lee). What should have been a simple pick up for the patient led to a week's worth of aggravation and suffering as he was at home coughing and going without his much needed medicine.

Please note, the patient had ample of time when this writer was on vacation to conduct the assessment. This writer immediately rendered a Step 3 violation and advised the patient that this counseling session will be placed on TEAM folder as the patient still non-compliant by completing the intake at Wheeler Clinic-IOP. The patient tends to make excuses that aren't legitimate as to why he cannot do the IOP. Please note, the patient was dishonest about attending his group sessions until this writer decided to call Wheeler Clinic of which he confessed about his dishonesty. This writer ensured to address her disappointment of the patient's non-compliance as it is part of his recovery process.
