If a child becomes ill while at A Child?s Paradise verbal authorization may be given to the director over the phone. The director will complete the medical authorization form and the parent must sign it when the child is picked up. When medication is given to a child the staff member that administers it must document the type of medication, when the medication was given, and how much. Documentation of Accidents: Staff/volunteer members must document all accidents that occur at A Child?s Paradise using the proper form. When filling out these forms staff/volunteer members must remain confidential and not use other children?s names, but the forms must be completed with as much detail as possible. All accident reports will be given to parents upon pick-up. If the accident is serious enough the parent may need to be notified before pick-up. Documentation of Special Health Needs: All children with allergies must have an allergy action plan posted where everyone can see. If the allergy is food related the allergy action plan must also be posted in the food preparation
