Currently I am a Licensed Physical Therapy assistant, I have practiced Physical Therapy for 10 years and after graduation I would like to take my boards and become CHES certified this year. I would like to continue to work on my education after graduation and becoming CHES certified. I am going to continue to work towards my Doctorate in Physical Therapy. I have many prerequisites to complete in math and science within the next three years before applying for Physical Therapy programs. Organization/agency: The Orthopedic Institute, Location Lake City Florida I like this internship because it aligns with my long term goals of working with post-surgical and non-operative orthopedic patients. This internship aligns with preparing me for a health care facility and would help me meet the requirements of a physical therapy program. 2. I am currently in the process of working towards meeting the internship requirements; I have my CPR AED and First Aid Certification through December of 2017. I have paid for my professional liability insurance last week. My goal is to devote time every day towards beginning and completing my internship for this upcoming summer. I am interested in the field of adults and older adults. Preferably in a clinical setting, I would want to complete my searches for internship sites by February 16th, I have one option from the list of internship sites and I am going to have to locate other possible candidates in my area and get them approved hopefully between February 23rd and March 2nd. I want to visit and meet the supervisors at the internship sites by March 16th. I want to make a final decision on internship sites by March 23rd. I would like to submit my internship application by March 30th to April 6th deadline. During these dates I will be putting together my resume and talking more to you Dr. Moses during these dates to make sure I am on the right track to prepare for my internship
