Triple aim initiative is an initiative launched by Institute for Healthcare Improvement (IHI) in October of 2007. The program was designed to help healthcare organizations achieve three healthcare objectives which being, improving the health of the populations served, enhancing the patient experience while reducing the cost of care. Three diverse healthcare organizations with different healthcare delivery models were included in this case study, they are, CareOregon, Genesys health system, QuadMed. The conclusions drawn from this case study would serve as a guide and help other healthcare organizations to implement newer healthcare delivery models while transforming their current models. In 2003, for the first time the word population health was used by David Kindig and Greg Stoddart, they defined population health as "the health outcome of a group of individuals, including the distribution of such outcomes within the group." (Pizzi, 2015). According to David Kindig, these groups are often geographic populations such as nations or communities, but can also be other groups such as employees, ethnic groups, disabled persons, prisoners, or any other defined group. Alternatively, in the context of a community, there are several existing social determinants of health, and they will have a substantial impact on individual health outcomes.
