Target Audience The CNE interact with the nursing staff on a daily basis and have information regarding the future plans for the unit, as a result, she was an indirect source of the staff’s learning needs and a valuable resource (Lockhart, 2006). The identified clinical problem was potential and actual communication breakdowns that occurred on the unit. Communication is important in all aspects of patient care but is comparatively more critical when there is a change in patient status or plan of care, during handoff or when providing information to the patient (Cornell, Townshend, Yates, & Vardaman, 2014; Hickman, 2015; Krautscheid, 2008). The key players during these interactions are nurses and: (a) physicians and allied health; (b) nurses; and (c) patients, respectively. Communication breakdowns that occur pose great risks to patient care, such as medical errors, adverse events, and ineffective nurse-patient and professional relationships (Abraham, Kannampallil, Almoosa, Patel, & Patel, 2014). Resultantly, they need to be addressed. By providing information on what communication breakdowns are and its consequences, the target audience will understand the importance of having and using a standardized communication tool (ISBARQ and SBAR3).
